This isn't really helpful for your problem, just a comment, but it really helps when you use punctuation. I can deal with no capitallization (that's how i used to be, i still forget it sometimes) but punctuation is very handy. Y'know, periods and commas and such.
As regards to your problem... the closest symbol the game has to a "little heart" is the blue arrow to indicate levitation. Some possibilities though: you may have stepped on a mine, and you may have had some wands explode. The polymorph may have been caused by a wand of polymorph, the teleport by a wand of teleport, but i can't think of anything that could cause enemies to levitate...

He might be referring to the little hearts denoting a "pet" or "companion." Those are the hearts I thought of.

I think a wand of cloning may have exploded on him, and his polymorph wands polymorphed all of the clones, and himself, in to various monsters. It happened to me once, but I only got 2 clones. I'm not sure how all the monsters would be tamed, if he's not exaggerating.

If I recall correctly, in earlier versions complex series of explosions could change your team to the monster team if you died and were resurrected immediately, or something like that. That would explain the hearts.
Also, I think savescumming or loading a crashed game can sometimes cause you to change identity like you explained, so that one of the monsters becomes the player and you can't retrieve your old form, and all monsters become your friends.
Historically, complex chains of explosions have indeed been bug magnets. We use to test them before every release by having it rain hostile mages, wands and explosives in Attnam and fighting there in WMode until we get bored. Yet we haven't found any such bug in 0.50.
If you have a save before or after such a crash, or both, we'd be very grateful if you could pack and upload them somewhere so we could debug them and fix the bug.

The doors appearing around you is normal; a wand of door creation just exploded in your inventory. If you truly can't polymorph back, it is a bug.
